Many women come to Bethany Womens Healthcare looking for something different. To accomplish this we often step outside the box of convention and courageously address systematic change with innovative approaches. Our goal is to seek strategies that empower all of our patients and offer them resources and support where they need it most.

Office Hours
To accommodate family schedules, we have early morning hours from 7:30 a.m. and late evening hours until 7:00 p.m.
Referrals/Prior Authorization
While it is the responsibility of the insured member to obtain a referral prior to an appointment, our office will assist in any way possible to see to it that proper referrals and other authorizations are received.

Note: If you do not have insurance, or your insurance has no maternity benefits, plase bring the following information so we can assist you in financial aid through a BABY ARIZONA application, done right here in the office:
- If you are a citizen, proof of citizenship (e.g. birth certificate or passport)
- If you are a legal alien, proof of your legal status (e.g. green card)
- Proof of residence (e.g. utility bill, piece of mail, rental agreement)
- Picture ID (e.g. driver's license, student ID)
- Verification of income for patient and financially responsible person such as parents or spouse, for past 30 days. (e.g. pay stubs, statement from employer if paid in cash)
- Verification (insurance card) of other insurance, if applicable
- Social security number
- Dependent care expenses for past 30 days if applicable (e.g. receipt from day care provider)
